Graduate forms for exceptional circumstances

Graduate Studies Office (GSO) forms are regularly updated. You should make sure that you are using the latest version by obtaining forms from this page ahead of each new application.

You may find yourself in exceptional circumstances that require appropriate adjustments to your study timescales and arrangements. Below are forms that should be used in such circumstances. It is strongly advised that, in the first instance, you discuss issues with your supervisor and College Advisor before beginning any of the below exceptional circumstance forms. If you are unclear on the submission/approval process for a particular GSO form or if you have questions about the sharing of your personal data, please contact your Graduate Studies Assistant or departmental graduate administrator.

For routine progression forms and graduate applications please refer to the Graduate forms webpage.

* Please note: a number of these previously paper-based forms have now been replaced by on-line applications available via Student Self Service and the links have been updated accordingly.
