The application process for the Accelerated Medicine course (A101) at Oxford consists of (1) completing this supplementary application form and supporting documents, (2) completing a UCAS application and (3) taking the Biomedical Admissions Test (BMAT). You must have submitted both forms and registered for the BMAT by the deadline of 6pm UK time on 15 October 2017.
For this course you are allowed to name two colleges of preference (if you wish). Please select the same as on your UCAS application for the first preference. If you have no preference, a college will be chosen for you in such a way as to even out the number of applications between colleges as much as possible (i.e., to ensure that each college has about equal numbers of applicants per place).
Some colleges are routinely oversubscribed: in an attempt to balance the number of candidates for each available place, some candidates might be reallocated to other colleges, even if they have expressed a first preference for a particular college.
In addition to completing this form you must also provide the following documents:
- Personal statement. Please provide a brief statement (of no more than one page) of your reasons for wishing to undertake medical training and for wishing to study on this course. Please make sure that your name and the course you are applying for (A101) are entered on this statement. You will also be asked for a personal statement as part of your UCAS application - the two statements can be the same if you wish, but you must still supply a copy separately.
- Academic transcript. Please provide an electronic copy of your transcript or other detailed official academic record of your higher education to date.
- Proof of English proficiency. If your native language is not English, please provide evidence of your English proficiency (see English language requirements for more information).
These documents should be emailed as PDF or Word attachments to Ms Marion Edney (firstname.lastname@example.org) no later than 15 October 2017. Please take care to include your candidate name in each document name.
The final questions on this supplementary form ask you to provide details of three referees. Please give the names and positions of three referees, at least two of whom should be familiar with your most recent academic performance.
You will also be asked for a reference as part of your UCAS application. This can be the same as one of your three references.
