The application form for 2019/20 entry is now closed.
The application process for the Accelerated Medicine course (A101) at Oxford consists of (1) completing this supplementary application form and supporting documents, (2) completing a UCAS application and (3) taking the Biomedical Admissions Test (BMAT). You must have submitted both forms and registered for the BMAT by the deadline of 6pm UK time on 15 October 2018. Please contact firstname.lastname@example.org with any queries.
In addition to completing this form, you must also provide a brief statement (of no more than one page) of your reasons for wishing to undertake medical training and for wishing to study on this course. Please make sure that your name and the course you are applying for (A101) are entered on this statement. You will also be asked for a personal statement as part of your UCAS application - the two statements can be the same if you wish, but you must still supply a copy separately.
Your personal statement must be submitted as a PDF or Word attachment as part of this applicant form.
The final questions on this supplementary form ask you to provide details of two referees. Please give the names and positions of two referees, at least one of whom should be familiar with your most recent academic performance. These referees should not be the same as the referee provided on your UCAS application.
You should ask your two referees to email their references as PDF or Word attachments to Mr Nicholas Pothecary (email@example.com) no later than 15 October 2018. Please ask your referees to include your candidate name in each document name.

If English is not your first language you may also need to meet our English language requirements.
If you have any questions about completing this form or about the application process, please contact Mr Nicholas Pothecary (firstname.lastname@example.org) or see the course website or Undergraduate Admissions page for further information.
